To Claim Rebate, Mail in the following:
- A copy of the sales receipt showing the dealer name, date and purchase price.
- Cut out and send in your original UPC code labels from each radio box.
- Complete the rebate form.

TERMS:
- Radios must be sold, delivered and invoiced to an end user during the promotion period (November 11, 2009 - March 31, 2010). Minimum quantity of 5 units, and a max of 50 units.
- A rebate check will be mailed to the end user within 6 weeks from submission of rebate request. All official rebate forms must be received by April 30th, 2010.
- An original rebate form and copy of the customer’s original receipt must accompany the rebate request. Model and serial numbers must be listed. Include Bar Code Label for each radio purchased. Forms that are not completely filled in or do not have the appropriate attachments will be voided and ineligible for the rebate.
- If there is not enough room to fill in your purchases, attach another form or list the additional purchases on the back of this form.
- Rebate offer is only for the radios listed above.
- Rebate for US Residents only from any authorized US Icom dealer.
